Thank you for your interest in becoming a Contractor with Centacare Home Safety Services.

Below you will find the information and documentation that is required in order for you to become a valued Contractor for our clients. Please read all the documents listed below and complete, sign and return to us.

Contractor Agreement

As a preferred contractor with Centacare Home Safety Services, you are required to enter into an agreement with us. As a registered contractor you then will be able to receive referrals for work requests on behalf of our clients. Please complete the relevant sections for your speciality, in particular, PART C – Contractor Rates.

Contractor Handbook

All staff who attend site for Centacare work requests are to read and comply with this document.

All staff who attend site for Centacare work requests are to read and comply with this document.

Contractor Induction Register

After all staff have read both the Contractor Handbook and the Contractor Induction Manual, please have everyone complete this register.

National Police Certificates

Contractors and their nominated field staff must hold a current National Police Certificate (which is valid for 3 years) and when presented to us, for renewal or for new staff, must be no older than 6 months from the date of issue.

Approved Contractor ID Card/s

Approved Contractor IDs will be issued to you at our meet and greet interview, once all documentation has been completed, returned and processed by us.

All your nominated staff must receive a Centacare Approved Contractor ID Card before attending Centacare work requests.

To assist us with this process, we ask that you provide a head and shoulders photo for all your nominated staff.

Meet and Greet Interview

There will be a need for us to meet with you to discuss our Vision, Mission and Values, our workflow, our clients’ needs and expectations.

Safe Work Method Statement

With recent changes to the Workplace Health and Safety Legislation and in light of Centacare’s recent analysis of this Legislation, Centacare request that, our contractors who complete elevated work, provide a Safe Work Method Statement.

In this document, we ask that you explain how you intend to safely complete elevated work. This will ensure that Centacare is caring for the safety and wellbeing of all its associated staff. This process will also enable us to work with you, and utilise our safety staff and expertise, to help keep us all safe in the workplace.

Centacare has determined that this process is required prior to us allocating your organisation any elevated type tasks.
