Centacare Hervey Bay Memory Health and Wellness Hub allows you to take some time out for yourself while providing new, enriching experiences for your loved one. This hub caters specifically to people who are living with dementia or other neurological condition, providing a safe, supportive and dementia-friendly environment. Enjoy the peace of mind that comes from knowing your loved one is experiencing the same high quality care they are used to along with the opportunity to exercise their independence in a small, group environment.
Whether it’s a balance class, music therapy, a spot of gardening in our purpose-built, raised garden beds or a gentle walk with our walking group, a day or two per week at our Hub will give your loved one the freedom to connect with friends and get involved in activities that suit their needs, interests and capacities. Your loved one will be treated to healthy, home-cooked meals each day – and we can cater for special dietary requirements. And if your loved one needs assistance getting to and from the Hub, our group transport service is available. Our hub operates from 10am-3pm each day, with pick-ups starting from 9am.
For carers with Queensland Health Dementia Respite (QHDR) funding, the out-of-pocket cost per day is $11.50, or $23.00 including transport to and from home.
Centacare has Queensland Health Dementia Respite (QHDR) funding available to support you to work, volunteer, study, or care for others if you care for:
You do not have to live with the person you care for to qualify for QHDR funding, and you don’t need a My Aged Care referral to access the service. The support you receive through QHDR can be in addition to what you receive through other programs including your Commonwealth Home Support or Home Care Package. To be eligible, carers must work, study, volunteer or care for others (i.e. dependants under 18 years).
The application process for QHDR is straightforward and we can assist you in quickly accessing additional support. Please call your Service Leader or our call centre on 1300 236 822 to find out more.
At Centacare, we are committed to supporting individuals living with a range of neurological conditions, including dementia, Parkinson’s disease, Multiple Sclerosis, and Huntington’s disease. Our support workers receive specialised training to help them understand the unique challenges posed by these conditions, learning strategies to communicate effectively and provide person-centred care.
We work closely with you to understand your loved one’s needs, whether related to dementia or other neurological diagnoses. What established routines bring comfort? How can we best manage transitions? We aim to create opportunities for meaningful connection, tailoring our support to what has been important in your loved one’s life. Our approach ensures we can provide not only high-quality care but also maintain the dignity and independence of those we support.
